Pathlights offers COVID-19 vaccinations for home-bound seniors

PALOS HEIGHTS, IL – Pathlights is now setting up COVID-19 vaccination appointments for homebound individuals. Pathlights, formerly known as PLOWS Council on Aging, works with AgeOptions and the Cook County Department of Public Health to offer home visits so that people who cannot leave their homes due to age, disability, or illness can be vaccinated.

Suburban Cook County residents may schedule a home visit for a COVID-19 vaccination if they meet any of the following requirements:

  • Are 65 years of age or older or have a disability that requires home help;
  • Use adaptable equipment such as ventilator / oxygen, crutches, walkers, wheelchairs, etc. and / or accessible means of transport to get out of the house;
  • Leaving the home is not an option as it involves a significant tax expense.

If you, a family member, friend, or neighbor meet the criteria, call the Cook County Vaccination Line at 833-308-1988, Monday through Friday from 7:00 a.m. to 10:00 p.m. and Saturday from 8:00 a.m. to 10:00 p.m.

After signing up, you will receive a confirmation email or phone call from one of the CCDPH’s COVID-19 vaccination provider partners. Please be patient as there are many inquiries.
