COOK COUNTRY, MINNESOTA July 18, 2021 (LSN) Cook County Public Health and Human Services (PHHS) and Sawtooth Mountain Clinic (SMC) have worked in partnership for over 20 years to offer a wide range of programs to the Cook County community.
This includes check-ups for children and adolescents, development screenings for toddlers (Follow Along), home visits to pregnant and postpartum families, outreach, the coordination of the Statewide Health Improvement Partnership (SHIP) and the Women, Infants, and Children (WIC). Nutritional support program. This longstanding relationship between the PHHS and SMC has been a strong foundation to build on during the COVID-19 pandemic – as we have worked together in new ways to serve the community.
Child and adolescent examinations
- Reminds families to bring children for regular medical and dental checkups
- Offers development screenings for children
Visit to the family home
- The SMC Parent-Child Health Nurse offers supportive visits to families and can speak on topics such as infant feeding, antenatal care and early childhood development
- Evidence-based home visits also provide long-term support for families
Range
- Nursing staff at the SMC provide foot care in the hub
- Clinical training on the correct installation and use of a car seat by a registered car seat technician
- Regular health education and communication on WTIP, in Northern Wilds, on the Topic of the Month and on SMC social media platforms
- Support for the children’s dental program through the Oral Health Taskforce
Nationwide Partnership for Health Improvement (SHIP)
- Provides community leadership for active living and healthy eating, such as the Safe Way to School program, community gardens, and the local farmers’ market
- Counteracts commercial tobacco consumption
Nutritional Support for Women, Infants and Children (WIC)
- Provides nutritional support to income-earning families that include a pregnant individual and / or child up to the age of five
- A peer lactation counselor provides educational support and encouragement to qualified breastfeeding parents
Together, PHHS and SMC are proud to offer this wide range of programs to support life-long community health and make Cook County a healthier place for all. This long-standing relationship has responded to various needs during the pandemic, such as providing planning assistance for companies and events so they can work more safely.
